Our goal is to get you home as quickly and safely as possible, so you can enjoy the family, activities and daily routines you love. With that in mind, discharge planning begins on the day you arrive. During your recovery, we’ll be assessing whether or not your home is an environment that is safe for you to return. If not, we’ll make recommendations for equipment, modifications and renovations that can make it safer.

We also work with many home health providers and can make recommendations for in-home care and assistance. Having a professional looking out for you after discharge provides a continuity of care and helps reduce hospital readmissions. We want to help you achieve your highest possible health and independence even after you leave our facility.
